Wabash Cannonball requires a minor release form and a liability release form for all riders under 18. Parents and guardians are the only people allowed to fill out the forms and give consent. Also, a parent or guardian must be present at the track in case an injury occurs and hospitalization is needed.
If a parent or guardian cannot be present at the track during a race, a notarized permission note must be provided. Bring two copies, one for the management, and one to take to the hospital in case of an emergency. Also, all contact information for the parents or guardians must be provided with the above forms.
Below is an example of a permission note.
My son/daughter, {Minor's name}, has permission to ride motocross at Wabash Cannonball on {Date}. I authorize {Guardian present at track} to be my son/daughter's legal guardian on {Date}, to sign all needed forms at Wabash Cannonball and to give consent to any necessary treatment.
